This Form is used to Judge the Community & School Activities worth 15% of your total Score as well as the Scholastic Achievement worth 15%. This form is Mandatory and is scored by the judges as part of our Scoring for the State Title. Please type this form.

PLEASE NOTE: Delegates are allowed to attach one additional page to the Competition Form, which accounts for a total of two one-sided pages. (Your first page will be your Competition form, and for your second page you may choose to continue the Competition Form in detail, or add a one-page resume.) Only one page double-sided or two pages one-sided in total will be allowed. DO NOT EXCEED THIS LIMIT. Pages that exceed this limit will be discounted and not factored into delegate's score.
